Remove beneficiary due to a will or assignment
If you have made a revocable nomination, use this option to remove your beneficiary when the nomination has been cancelled by a will or because the policy has been assigned to someone else.
Before you begin
Please ensure that:
- You are the policy owner.
- You are at least 18 years old.
How to submit your form
- Download and complete the Notice of Revocation of Revocable Nomination Form.
- Print the form and obtain the policy owner's signature in wet blue ink.
- Mail the completed original hardcopy form to us via the following address:

Use this option if your beneficiary is being removed because:
- You have made a will that cancels your existing beneficiary arrangement; or
- You have assigned ownership of your policy to another person.
The policy owner is required to sign the form. Ensure the signature is provided in wet blue ink.
- Requests received before 3pm will be processed in 3 business days.
- Requests received after 3pm will be treated as received on the next business day.
If your request requires additional review or special handling, processing may take longer. We will contact you if further information or supporting documents are required and keep you updated on the status of your request.
