Change of address forms

1. Sign in to your plan on the website.
2. Under My Account, click My Personal Information.
3. Follow the prompts to update your information.

As an insurance customer who purchased a policy independent of any other organization (e.g., you bought insurance through an advisor), you may be able to change your address online by:

1. Visiting the Personal insurance web portal to, first, confirm if your insurance policy is accessible online.
   If it is, you can then log in and update your address (Canadian addresses only).
2. If you cannot access your insurance policy online, you can complete the Change of address form (PDF), and send it in to us by email, fax or regular mail. 

Find out whether you should change your personal information (i.e., address) with us or through your company’s HR team.

Sign in and go to ‘My profile’, ‘View my plan material’ or call us at 1-888-727-7766, Monday to Friday, 8 a.m. to 8 p.m. ET.

If your plan lets you change your personal information online:

Sign in and go to ‘My profile’, ‘View my member information’.

Otherwise:

Fill out a form for the plan on which you want to change your information (they are PDFs and may download to the lower left corner of your screen):
Request for change - RRSP, TFSA, LIRA, RPP, NRSP (PDF)
Request for change - RRIF, LIF, LRIF, PRIF, RLIF (PDF)

Then send us the completed form to us.
